Ranked first for back pain is the Eve the wunderflip ultimate hybrid sleep mattress, at 5.0 / 5 on the reweighted rubric. The Staples Revitalise Eco Latex Pocket 3800 Mattress sits just under at 4.9 / 5. Entries on the shortlist span £999.99 through £2,099.99.

Back pain doesn't usually yield to one fix, and the mattress is only ever a part of it. What the rubric can do is rank what owners keep reporting on comfort and pressure relief, because those are the two categories the trial evidence lines up with most reliably. It won't tell you why your back hurts. Nothing on a listing page will. What follows is filtered to mattresses the manufacturer positions for back-pain sufferers, and it refreshes overnight as new reviews land.

Different bodies want different defaults, and that's the part the marketing skates over. If you're heavier, if you sleep on your side, or if you're managing a specific spinal condition, you'll want to sit off the central baseline. That baseline comes from one clinical reference worth knowing about: the Kovacs randomised controlled trial published in The Lancet in 2003 (doi.org/10.1016/S0140-6736(03)14792-7), which compared medium-firm against firm mattresses in adults with chronic non-specific low back pain. Medium-firm came out ahead on pain in bed, pain on rising, and disability at ninety days. For ranking this list, separate from the unweighted BedBoy Score that appears on each product page, comfort and pressure relief carry forty per cent extra weight and edge support twenty per cent extra, because those three categories track most closely with what reviewers say after sleeping on the product.

It's fair to ask what the ranking rests on. None of the mattresses below have been slept on by an editor, because BedBoy doesn't run that kind of testing. A product gets there when hundreds of independent reviewers agree on the same categories. Where the sample isn't deep enough to draw on, that category stays empty. None of this displaces a GP appointment if pain is severe or persistent, or carries red-flag symptoms. The NHS public-health overview covers the self-care steps and the warning signs to watch for.

How we ranked this list

Comfort and pressure relief carry forty per cent extra weight on this page, and edge support twenty per cent extra, because back-pain reviewers cite those categories most often in verified reviews.

Category weightings, source data and the full rubric are documented on the how we score page.

What the NHS says about mattresses and back pain

The NHS public-health page on back pain doesn't single out one mattress type as the answer. Its standing advice covers staying active rather than resting in bed, gentle low-impact exercise like walking, swimming or yoga, over-the-counter painkillers used in line with the leaflet, and a GP appointment if pain is severe, fails to improve after a few weeks, or comes with red-flag symptoms like numbness in the legs or loss of bladder control.

Inside that frame your mattress is one of several things you can change. So is how much you move during the day, the chair you sit in at work, what you're carrying, and any position you sleep in that traps a nerve for eight hours. A medium-firm sleeping surface that supports the natural curve of the spine without bowing under it is the sensible starting point for most adults. If yours is visibly dipping in the middle, or you're waking up sorer than you went to bed, that's your answer and no amount of persevering will fix it, and the NICE guideline on low back pain (NG59) treats the sleeping surface as a modifiable factor in self-management.

Frequently asked questions

What firmness is best for lower back pain?

Medium-firm, for most adults. That's where the trial evidence settles for chronic non-specific low back pain, and it's the answer I'd give you before I knew anything else about you. Go softer and your lower back drops under its own weight all night. Go very firm and your spine sits straighter than it wants to while your hip and shoulder take the pressure instead. If you're heavier, take a step up from medium-firm. If you're light, you can usually take a step down without losing anything.

Should I pick memory foam or pocket sprung if I have back pain?

Neither construction wins outright for back pain. What fits depends on your build, how you sleep, and how warm you run. Pocket-sprung mattresses give clearer point-to-point support and a defined feel; they tend to score higher on edge support, which counts for a lot if you sit on the side of the bed in the morning. Memory foam contours more closely to the body and can relieve pressure at the hip and shoulder for side sleepers, but it sleeps warmer and is harder to move on. Hybrids (a pocket-spring core with a foam comfort layer) appear most often on the back-pain shortlist because they pair the support of springs with the contour of foam.

How thick should a mattress be for back pain?

Twenty-five to thirty-two centimetres suits most adults. That's deep enough for a comfort layer that relieves pressure, on a support core that won't bottom out under you. Mattresses under twenty-two centimetres can compress through the comfort layer under heavier sleepers and transfer load to the spine. Above thirty-two centimetres adds little for back pain alone and can make getting in and out of bed harder. If you wake up stiff, how easily you can get off the thing in the morning will do more for you than another three centimetres of foam.

Is a firmer mattress always better for back pain?

No, and this is the one I take out of other people's copy most often. Buying firmer to fix your back is the classic mistake, and the trial evidence points the other way for most adults. Too firm holds your lower spine straighter than it naturally sits, which takes away the support those muscles want overnight, and your hip and shoulder end up carrying load they weren't built for. Side sleepers feel it worst. Plenty of people wake up sorer on the firm mattress than on the softer one they got rid of. Firm suits a minority: heavier adults, predominantly back or front sleepers, or those under specific clinical advice.

How long does it take a new mattress to ease back pain?

Two to three weeks, not two or three nights. Give it three nights just for the mattress to settle under you, which is the minimum both manufacturers and reviewers quote, and then give your body another ten to fourteen to catch up with it. A bit of stiffness in the first week is normal and usually passes. Pain that gets worse, or that hasn't settled after a fortnight, means the firmness is wrong for you rather than that you need to try harder. Retailers who offer a hundred-night home trial have priced that uncertainty into the product, so keep the receipts and the packaging until you've decided.

Methodology in brief

Each composite comes from manufacturer spec sheets, live retailer pricing and pooled owner reviews. Nothing on any of these lists has been slept on or handled by an editor. Category scores and the source for every number live on the how we score page.

Brands can't pay for placement, and the affiliate networks don't see the running order before it goes out. When the inputs change on the nightly refresh, the order moves with them.

Still weighing up the options?

If the shortlist above hasn't settled it for you, the mattress quiz narrows the whole catalogue by how you sleep, your build and your budget, and hands back something shorter.
